Assisted Living Facilities
Many assisted living facilities can benefit greatly from a social worker. Our social workers can come in on a contract basis to provide services regularly or as needed.
A social worker's role in assisted living facilities can include:
- Conducting psychosocial assessments upon admission or return from a hospitalization.
- Administering Depression and/or Cognitive Assessments.
- Meeting with families to address care or any other issues.
- Working with nurses as an interdiciplanary team.
- Addressing adult protection concerns.
- Assist tenants and/or their families with paperwork for county programs.
A few hours per week of social services can result in lower tenant turnover.
