Our licensed temporary social workers have all had experience working in Long Term Care settings. They can assist facilities with:

• Psychosocial Assessments (including Depression Assessments and/or Mini Mental Status Exams)
• Development of Social Service Care Plans
• Facilitating Care Conferences and Family Meetings
• Discharge Planning
• Admissions

All the social workers are use to working as team players. Very little training is needed!

 

Survey Readiness Assessment
