Housing Services Coordinator – scattered site
Temp Social Work is currently seeking an experienced case manager to fill an opening with a Housing Services Coordinator – scattered site located in the Twin Cities. The ideal candidate should be able to perform the duties of a nursing home social worker.
Starting rate of $28-$33/ hour (If you do not meet the criteria, the rate may differ)
Requirements
Bachelor's or Master's degree in Social Work from an accredited school
Valid Social Work License in the state of Minnesota
One year of experience working in a nursing home
Knowledge of community based services - Preferred

Responsibilities
Engage with participants to assess strengths and barriers and develop individualized, participant-driven housing stability plans.
Maintain accurate, timely documentation including goal plans, case notes, incident reports, funder data, receipts, and other required records.
Collaborate with the Housing Coordinator to transition tenancy-sustaining services to the Resident Services Coordinator as appropriate.
Provide tenancy-sustaining services through in-home and community-based support.
Conduct monthly apartment visits to ensure units are safe, properly maintained, and in compliance with housing expectations.
Assist participants with the housing recertification process.
Utilize problem-solving skills and community resources to support crisis prevention and intervention.
Connect participants to physical health, mental health, and recovery resources to support overall wellness.
Assist participants in maintaining or increasing income through benefits assistance, volunteering, education, and employment opportunities.
Support residents in building and strengthening social supports with family members, peers, neighbors, and community connections.
Coordinate or facilitate translation and interpreting services when needed.
Participate in professional development, team meetings, case consultations, special initiatives, and organization-wide meetings.
Preferred Experience / Strongly Desired:
Experience working with individuals experiencing homelessness is a significant plus.
Familiarity with county and community resources, including housing navigation and relocation support.
Experience supporting individuals with mental health conditions and/or disabilities.
Knowledge of local benefits systems and community-based services that support housing stability.
