MnCHOICES Assessor – Remote/Hybrid County Contract Position
$30–$35/hour + Mileage Reimbursement | Full-Time & Part-Time Options Available
Temp Social Work is currently hiring MnCHOICES Assessors to support a county partner in Minnesota. This is a great opportunity for an experienced assessor looking for a flexible role with a manageable workload, meaningful client interactions, and the ability to complete assessments remotely or in the community.
This position is expected to continue for at least one year, with the possibility of extension.
Position Details:
Full-Time: Approximately 10–12 assessments per month
Part-Time: Approximately 5–7 assessments per month
Schedule: Flexible scheduling options available
Work Setting: Assessments may be completed virtually or in person (some in person visits in Chisago County)
Pay: $30–$35/hour based on experience
Mileage reimbursement provided for required in-person visits
Responsibilities:
Complete MnCHOICES assessments with individuals and families
Gather information related to client needs, strengths, goals, and support systems
Complete required documentation accurately and within established timelines
Collaborate with county staff, service providers, and community resources
Assist individuals in connecting with appropriate programs and supports
Maintain accurate case notes, assessments, and related documentation
Qualifications:
Current MnCHOICES certification required
Experience completing MnCHOICES assessments preferred
SSIS experience preferred
Knowledge of Minnesota programs including: Medical Assistance (MA), Special Needs BasicCare/related programs, Service needs and support planning, and CFSS (Community First Services and Supports)
Strong assessment, documentation, and communication skills
Ability to work independently while managing deadlines and client needs
